Clinical documentation guide

Three clinical note checks: age, laterality and negation

A focused accuracy check for patient age, left versus right, and words that reverse the meaning of a finding.

The short answer

Before copying a note, verify the patient’s age, the anatomical side and each important negative finding against the encounter and transcript. Correct any mismatch explicitly. A single number, “left” or “no” can change the meaning of the case.

Check age without inferring it from a date

A dictation may contain an age, a date of birth, both or neither. Check the intended age against the correct encounter. If the sources disagree, resolve the difference rather than choosing the number that sounds most plausible.

Do not turn a misheard year or unrelated date into a patient age. The doctor decides what identifier and age information belongs in the official record under local policy.

Trace the anatomical side through the note

Trace left and right through the whole note, from the injury and symptoms to examination, imaging, procedures and plan. It is possible for the note to begin with a left-sided injury and later describe treatment on the right. Bilateral findings need their own check.

If the source contradicts itself, keep the uncertainty visible for clinician correction; do not let the model harmonise the text by guessing a side.

Read every “no” and “not yet”

“No bony tenderness”, “no fracture on X-ray”, “X-ray not performed” and “fracture not excluded” have different meanings. Verify the context of each negative. Also check “denied”, “declined”, “pending”, “possible” and “resolved”.

This review is faster when done against the full transcript, not just the polished note. A concise summary may hide a flipped negation.

Common questions

What if the transcript has two different ages?

Confirm the correct age from the encounter and correct the source before finalising the note.

Can the model resolve left versus right automatically?

Do not rely on an inference. The clinician must verify the correct side and correct any contradiction.
